The Workplace Safety Inspection Form (PDF) assists as a guide for the Joint Loss Management Committee (JLMC) Members when performing annual site inspections of Town buildings and by department heads/managers annually to identify possible safety hazards in their areas. Completed forms can be sent to the Human Resources Department to the attention of the JLMC Chair when completed.
